Non-urgent advice: How to Register

If you live within the surgery catchment area and wish to register there are a couple of options available:

  1. Register with a GP Online Service
  2. Register in person at the surgery reception by requesting a patient registration form

The GP Online Service is provided by NHS England. This is a secure site to enable us to register you and transfer your existing medical records. Once we have recieved and processed your registration, the surgery will contact you with confirmation.

Non-urgent advice: How to Temporarily Register

Should you require medical advice and temporarly resident in the surgery catchment area you are able to temporarly register with the surgery. You will need photographic ID to register. There are two ways to temporarly register

  1. Print and return the Temporary Service Form (GMS3)
  2. Register in person at the surgery reception by requesting an tempoarary registration form (GMS3)

Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records. The NHS has an online service to help you Find your NHS number

Each patient is allocated a named accountable GP. This is a contractual requirement. When you register you will be notified of your named accountable GP. If you are unsure who your accountable GP is and would like to know please contact the surgery. Patients are still welcome to see any GP or healthcare professional of their choice
